NEW PATIENTS

The physicians and staff at Richmond Pediatrics welcome new families to our practice including:

Parents expecting a new baby
Families needing to change from their current physician
Families new to the greater Seattle area
Families looking for care from a pediatrician-specialist (as opposed to a family practice physician)

We accept virtually all private insurers (except managed care) and always accept all qualified patients on the Washington State Healthy Options/DSHS program. You may want to facilitate your first visit to the office by completing our Demographic, HIPPA, Family and Social History Forms prior to arrival.

The Records Transfer Form authorizes your previous physician to release pertinent medical information to our office.

Our Patient Information Booklet briefly summarizes much of the information you might want to know regarding our physicians, our office and billing procedures and other general (non-medical) information.
